Job Summary:
As a member of the Integrated Care Team, the Care Treatment Coordinator facilitates coordination of care for members on all health plans. Ensures implementation of services to members in accordance with their health plan contractual requirements and reviews progress. Coordinates with involved healthcare, behavioral health and wellness providers, while maintaining current documentation on members’ progress and coordination of care efforts. Manages medical and psychiatric conditions, while assisting member in achieving their recovery goals.

ESSENTIAL JOB DUTIES AND RESPONSIBLITIES
  • Accepts referrals from sites or Member Engagement Team for new members. Works directly with assigned Psychiatric and Primary Care team to engage, educate, communicate, and coordinate care with service members, their family, behavioral health, medical and dental providers, community resources, health plan liaisons and others to ensure that all services in the Integrated Service Plan (ISP) are implemented.
  • Updates and maintains required medical record documentation.
  • Responsible to maintain an abbreviated Case Manager (Supportive/ Connective Level of Care) assigned caseload.
Assessment and Service Planning
  • As warranted by the health plan: Completes an annual Integrated Comprehensive Assessment (ICA) and updates it, as necessary.
  • Reviews the ICA with the Psychiatric Services and Medical Assistance staff and works collaboratively with the member and integrated care team when additional services are needed.
  • Develops and adjusts the Integrated Service Plan (ISP) based on identified needs and the preferences of the member for their specified health, wellness and behavioral health goals.
Coordination of Care
  • Facilitates Coordination of Care with all health plan liaisons
  • Ensures plan liaisons complete required coordination of care needs
  • Provides assistance and resources to support member access to covered behavioral health and healthcare services and community resources.
  • Coordinates and communicates with medical and psychiatric practitioners for individuals in inpatient levels of care or experiencing complex medical and/or psychiatric conditions.
  • Crisis support, including coordination with site and external crisis providers.
  • Participates in integrated care team meetings and documents actions to modify service delivery based on progress, social determinants of health, health literacy, current gaps in care, and level of self-management of chronic conditions.
  • Collaborates with Copa Health Care Management to identify and close gaps in care.
  • Coordinates with evaluation agencies for members referred for SMI determination.
Administrative Duties
  • Serves as the primary point of contact for the caseload.
  • Documents all coordination of care efforts and contact in electronic health record.
  • Complete required reports and spreadsheets, including Court Ordered Treatment (COT) tracking.
  • Maintains the person’s comprehensive clinical record, including new referral paperwork and documentation of activities performed as part of the service delivery process (e.g., assessment(s), provision of services, coordination of care, discharge planning, etc.).
  • Maintains documentation efforts consistent with Copa Health engagement expectations.
  • Ensures the development of discharge and aftercare plans prior to discontinuation of behavioral health or primary care services.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
Minimum Qualifications
  • Bachelor’s degree in behavioral health related field and 2 years’ of full-time behavioral health work experience; or
  • Bachelor’s degree in non-behavioral health related field and at least 4 years’ of full-time behavioral health work experience; or High school diploma or high school equivalency diploma and 6 years’ of full-time behavioral health work experience
  • Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
  • Valid AZ Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements.
  • TB Test clearance required at the time of hire and annually.

Knowledge, Skills, Ability

  • Knowledgeable of the SMI and GMH/SU systems, eligibility and services continuum(s).
  • Knowledge of various AHCCCS Complete Care (ACC) plans, the Regional Behavioral Health Authority (RHBA), non-Regional Behavioral Health Authority (RBHA) plans (i.e. ALTCS EPD/ DDD; SMI/ DDD) and contractual requirements and expectations for all.
  • Demonstrated ability to remain calm and use the proper de-escalation methods in crisis situations.
  • Proficiency in typing, using computer software, i.e., EHR/EMR systems and MS Office as well as the ability to learn new software systems as applicable.
  • Ability to maintain confidentiality and adhere to all HIPAA regulations.
  • Demonstrated ability to communicate, problem solve, and work effectively in an individual and team environment.
  • Must complete training and demonstrate competence in Integrated Care skills.
  • Effective interpersonal skills.
  • Good written and verbal communication skills.
  • Knowledge of psychiatric conditions.
  • Sound judgment.
  • Ability to follow directions.
