Who We Need
We are seeking a Referral Specialist to join our team. Accept and process external and internal agency referral packets. Work closely with program and eligibility staff to determine suitability of referral for intake. Communicate effectively with individual requesting services or with the referring agency. Initial entry and continued maintenance of client demographics. Identify any discrepancies in the referral packet and request any outstanding information. Communicate to program staff when a referral is ready to be scheduled and an intake completed.
MINIMUM QUALIFICATIONS:
- Requires any combination of training, education, and/or experience equal to a high school diploma or GED
- (General Equivalency Diploma).
- Minimum one (1) year experience in EHR Systems support, customer service, internal tracking and
- Experience working with individuals with disabilities. Ability to obtain and maintain a Level One Fingerprint Clearance Card.
- Valid AZ Driver’s License and MVR clearance is required. These positions do require driving company vehicles to transport members.
- Successfully complete a pre-employment drug screening.
- Must be able to lift minimum 50 pounds, stoop and bend comfortably in all positions.
JOB SUMMARY (Including, but not limited to)
- Receive and validate all agency referral packets to ensure that all necessary information is received and meets specific program intake requirements.
- Data Entry: Enter into the Electronic Health Record (EHR) Client Demographics, Basic Data Contact and Referral Demographics in a timely and accurate basis.
- Demonstrate excellent oral, written and electronic communication.
- Ensure the coordination and confidentiality of communications within the agency and outside referring agency in accordance with HIPAA guidelines.
- Demonstrate and maintain professional and productive working relationships with all agency departments, staff, board members, and community.
- Demonstrate excellent work habits to include time management, organizational skills, attendance, punctuality, and completion of work assignments.
- Ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
- Other duties as assigned.
