The CBR worker will provide support and rehabilitation services to people with disabilities, their families, and communities. Built rapport with community workers, Asha/Anganwadi worker/Health worker, conduct awareness programmes. The goal is to promote social inclusion, equality, and empowerment of people with disabilities. 1. Assessment and Identification: Identify people with disabilities in the community and assess their needs. 2. Rehabilitation Planning: Develop personalized rehabilitation plans with the individual, family, and community. 3. Community Mobilization: Raise awareness about disability rights and inclusivity. 4. Capacity Building: Train community members, families, and individuals with disabilities. 5. Referral and Linkage: Connect individuals with healthcare, education, and social services. 6. Advocacy: Promote accessibility and inclusivity in community infrastructure and services. 7. Monitoring and Evaluation: Track progress, identify challenges, and adjust strategies. 8. Collaboration: Work with community leaders, healthcare providers, and organizations. 9. Documentation: Maintain records and reports. Qualifications: Diploma/Degree in social work or rehabilitation, Experience: 1-2 years in community-based rehabilitation or disability work. Skills: Communication, empathy, advocacy, and community mobilization. Location: South kamrup, Mirza, Chayygaon, Boko Other requisites: 1. Experience working with community-based organizations. 2.
