Job Summary:
The LPN, Long Term Care (LTC), is responsible for providing professional nursing care to long-term care residents within a healthcare facility. They collaborate with other healthcare professionals to ensure the highest standard of patient care and wellbeing. The LPN, Long Term Care (LTC), administers medications, monitors patients' health, assists in daily activities, and communicates with families and healthcare team members.
Responsibilities and Duties:
1. Provide direct nursing care to long-term care residents under the guidance of registered nurses.
2. Administer medications and treatments as prescribed by physicians.
3. Monitor patients' health conditions and promptly report any changes to the registered nurse.
4. Assist residents in daily activities, such as bathing, dressing, and eating.
5. Collaborate with interdisciplinary teams to develop and implement individualized care plans.
6. Maintain accurate and up-to-date documentation of resident care and progress.
7. Ensure residents' safety by following established protocols and policies.
8. Educate and support residents and their families regarding care plans and health management.
9. Respond to emergencies and provide immediate nursing care as required.
10. Follow infection control and safety procedures to prevent the spread of diseases.
11. Help maintain a clean and organized environment within the long-term care facility.
Qualifications and Skills:
1. Valid LPN license in the state of [Insert State].
