Job Title: Long Term Care (LTC) Specialist
Job Summary:
We are seeking a dedicated and compassionate Long Term Care (LTC) Specialist to provide exceptional care and assistance to individuals in long term care facilities. As an LTC Specialist, you will play a vital role in ensuring the physical, emotional, and mental well-being of our residents. Your primary goal will be to enhance their quality of life by addressing their unique needs and providing personalized care.
Responsibilities and Duties:
1. Provide day-to-day care and support to residents in long term care facilities.
2. Assist with activities of daily living, including bathing, dressing, grooming, and toileting.
3. Administer medications and monitor the overall health status of residents.
4. Collaborate with healthcare professionals to develop and implement care plans.
5. Foster a safe and comfortable environment for residents.
6. Assist in maintaining proper hygiene and cleanliness in resident rooms and common areas.
7. Monitor and report any changes in residents' behavior, physical condition, or emotional well-being.
8. Offer emotional support and companionship to residents.
9. Engage residents in recreational activities to promote social interaction and stimulate cognition.
10. Collaborate with families and provide updates on residents' progress.
11. Attend training sessions and stay updated with the latest advances in long term care practices.
12. Adhere to ethical standards and ensure residents' confidentiality and privacy.
13. Comply with all safety and health regulations to maintain the well-being of residents and staff.
14. Maintain accurate documentation and records of residents' care, progress, and any incidents.
15. Participate in team meetings and contribute to problem-solving and quality improvement initiatives.
Qualifications and Skills:
1. High school diploma or equivalent.
2. Completion of a Certified Nursing Assistant (CNA) program.
3. Valid certification as a CNA or other relevant healthcare certification.
4. Previous experience in long term care or a similar care setting is preferred.
5. Excellent communication and interpersonal skills to build meaningful relationships with residents, families, and staff.
6. Passion for providing quality care and improving the lives of others.
7. Ability to work effectively in a team and in a fast-paced environment.
8. Strong problem-solving skills and ability to adapt to changing situations.
9. Empathy, patience, and compassion towards individuals with diverse needs.
10. Knowledge of basic medical terminology and understanding of medical procedures.
11. Ability to perform physical tasks such as lifting and transferring residents.
12. Proficiency in using computers and electronic medical record systems.
13. Flexibility to work various shifts, including weekends and holidays.
14. Knowledge of infection control procedures and safety protocols in long term care facilities.
15. Current CPR and First Aid certification.
