Assistant Case Manager

Job Summary:
The Assistant Case Manager's duties include administrative and supervisory duties for youth in the CFS system and young adults with an intellectual disability and/or struggle with mental illness. Areas of responsibility may include the following programs; Outreach, Cluster Apartment Support Services and Crisis Management.
Reporting to the Case Manager, the Assistant Case Manager is responsible for overseeing the support and services being provided to a designated caseload and the staffing team(s). At the direction of the Case Manager, this can include support plan and safety plan development; supervision (training, orientation and all aspects of performance management); communication with all collaterals, including FSH, Justice, family, and other professionals; and ensuring all K&D (Ltd.) facilities are well maintained to both Government and Agency standards.
Lastly, the Assistant Case Manager is scheduled into an on-call rotation for the Agency and must have the ability, skills, and knowledge to attend to crises. This includes being available to respond in person to an emergency. The Assistant Case Manager will be required to fill open direct support shifts when needed.
Position Requirements:
  • Must be 18 years of age or older.
  • Cover letter and updated resume.
  • Criminal Record Search Certificate with Vulnerable Sector Screening (no older than 3 months)
  • Child Abuse Registry Check (no older than 3 months)
  • Adult Abuse Registry Check (no older than 3 months)
  • First Aid/CPR Certification.
  • A minimum of 5 years experience providing services to young adults living with an intellectual disability and/or struggling with mental illness.
  • A minimum of 2 years of management experience in any industry. However, social services is preferred.
  • Experience in field supervision, and program development is considered an asset.
  • 2 applicable employment references.
  • Class 5F Driver's License.
  • Must have a reliable vehicle that can be used for work purposes.
  • Must produce a Driver's Abstract.
Minimum Educational Requirements:
  • Minimum of grade 12 or GED.
  • Post-secondary degree, diploma, or certificate related to human services is considered an asset, but not required.
Position Benefits:

Brief Summary of Duties and Responsibilities
  • Employ a person-centred approach with individuals receiving support
  • Maintain an awareness (and record) of the individual’s known diagnoses and symptomatic behaviour.
  • In conjunction with the support team, engage individuals in a process of goal setting, planning, and acquisition as it pertains to living safely and independently in the community.
  • In consultation with employees, develop and update individualized support and safety plans for each individual, using available data about the individual. Support plans to reflect the individual’s mental health/pertinent diagnoses.
  • Assist staff with or direct a process of safety plan development.
  • Develop, implement and oversee, independent living and recreation/leisure programs.
  • Assist employees with problem-solving around providing direct service to individuals.
  • Oversee and maintain an awareness of cumulative health care of the individual (including general practitioner, dentist, ear and eye specialists, and psychiatrist appointments).
  • In collaboration with the Case Manager, address needs about the individual transition as necessary. (Social Workers, family members, etc.)
  • Collaborate with Case Manager concerning staffing and scheduling as required.
  • Supervise and schedule (as needed) employees with assigned individuals/departments.
  • With the support of the Case Manager review and follow up on incident reports.
  • When required, contribute to Red Ladder assessments.
Brief Summary of Administration Responsibilities
  • Maintain up-to-date files on individuals (static and dynamic information) as well as Outreach worker supervision (if applicable).
  • Maintain up-to-date documentation on employees' performance.
  • Maintain daily process notes on employees’ activities and multi-system interactions.
  • Maintain annual and bi-annual reports on individuals.
  • File incident reports in collaboration with the support team and Case Manager.
  • Maintain an accurate, up-to-date record of each individual’s medications and other medical treatments or procedures.
  • When requested, complete a facility audit.
  • Ensure that month-end is audited and turned in on time.
  • Ensure that payroll is submitted accurately and on time.
  • Any other duties assigned by the Case Manager.
Skills
  • Strong Attention to detail and organization skills.
  • Strong oral and written communication skills are a must.
  • Self-starter and proactive approach to resolving problems and issues.
  • Able to use effective troubleshooting and problem-solving skills.
  • Computer literacy skills minimally must include the ability to use all Microsoft Office programs (i.e. excel, word).
  • Ability to draft correspondence in a professional matter for both internal and external stakeholders.
