Primary objective
Deliver effective pre-release planning with sentenced prisoners (including individuals with multiple barriers to successful
reintegration) using an integrated through care / case management approach.
Key duties and responsibilities
1. Develop and implement collaborative through care plans (using a case management model) enabling effective prisoner
management and re-engagement of prisoners with their community and targeted community supports.
2. Contribute to individual Offender Management plans by developing collaborative individual through care plans for the
transition period from prison to the community.
3. Maintain accurate, timely and thorough prisoner records.
4. Actively motivate prisoners to participate in the reintegration process and achieve through care goals maintaining a focus
on the reduction of recidivism.
5. Maintain strict privacy and confidentiality at all times with all prisoner information and records
Selection criteria
Essential
1. Demonstrated high level of communication skills and abilities to provide services to people from diverse cultures,
particularly Aboriginal Australians.
2. Demonstrated high level administrative and organisational skills including flexibility and resilience in approach to high
demand workloads; a high standard of written and oral communication skills; and workplace reliability in maintaining key
outcomes.
3. Demonstrated ability to record and maintain client/prisoner records to a high standard and in a timely manner;
maintaining record security and prisoner privacy.
4. Experience in the development and implementation of comprehensive case plans with individuals presenting with
complex social and/or other needs that facilitate safe and effective transition to community.
5. Demonstrated ability to liaise with a wide range of Elders, Respected Persons, Government and Non-Government
agencies to support prisoner pre and post release planning.
6. Knowledge of principles and theories relating to offender cycles of behaviour to maintain prisoner engagement to
achieve goals; identify barriers to reintegration and promote community safety.
Desirable
1. Current NT driver’s licence.
2. Relevant qualifications in social welfare, Indigenous studies, or associated discipline.
3. Previous experience in a custodial environment.
