With two offices located in Katherine & Darwin, JMJ Disability Serv Pty Ltd is looking for experienced permanent full-time Disability Services Officer (DSO). Annual salary ranges from $88,000 - $108,000 relevant to their qualifications and demonstrated experience. Qualifications & experience · Minimum Diploma or higher obtained in Australia in the health or welfare (Preferably Psychology or social work) with minimum 1-year full-time work experience in coordinating supports to people with disability; OR · Relevant overseas qualification equivalent to Diploma or higher (Preferably Psychology or social work) with minimum 2- year full-time work experience; OR · Minimum 3-year full-time work experience if no relevant qualification o Current Firs Aid Certificate o Unrestricted full driver's license o Cleared NDIS Worker Screening o Cleared police check Purpose of Role Responsible for maintaining a positive and effective team environment and the coordination of support to service users accessing the company’s services. Assess/Interview, manage, plan, monitor and report on service delivery to new and existing service users and to develop positive relationships with service users, families, and other stakeholders to achieve quality outcomes for service users. The DSO will assess the need of a client including community linkages, mainstream, informal, and funded support. Key Responsibilities · Assess referrals and coordinate the analysis of support need, planning, and program development for service users. · Will support the client/s to understand and implement the funded supports in your plan and link the client/s to community, mainstream, and other government services. · Will assist client/s to negotiate with service providers about what they will offer and how much it will cost out of the NDIS plan. DSO will ensure service agreements are completed and will help build the client/s the ability to exercise choice and control, to coordinate supports, and access local community. · Will assist client/s to 'optimise' their NDIS plan, ensuring that client/s are getting the most reasonable and necessary funded supports. · Coordinate and ensure the development of client/s’ support plan (including supervising/supporting key workers in developing Individual Plans). · Coordinate rostering requirements according to individual support needs of client/s and service agreements. · Ensure effective communication around the company service provision with families and key stakeholders including government agencies. · Communicate with line management about program and staffing requirements. · Provide after hours on call support to teams. · Fulfil any other duties as required.
