If you are successful, you will work in a Supported Independent Living (SIL) environment to provide people with high complex support needs, an opportunity to live in an environment that supports and maximizes personal growth and independence. Scope The Disability Support Worker will: Provide assistance for people living with a disability and/or complex needs, to remain living independently in their own homes and community, in a dignified and safe manner in accordance with individual care plans. Provide monitoring of participants health and well-being and contributing to service development. Essential Duties and Responsibilities Provide culturally appropriate direct support to service users in the following Key Responsibility Areas:
Domestic Assistance
Personal Care
Respite Care
Social support
Transport
Medication monitoring
Establish appropriate and effective working relationships with service users, maintaining professional boundaries.
Provide quality service at all times by responding to individual needs and requirements with flexibility and safety.
Assist the service user in maintaining and enhancing practical skills in order to promote their level of independence.
Complete and maintain documentation in accordance with organisational policies and procedures, and quality standards requirements and contribute to continuous improvement within the agency.
Observe and report any changes in the service user’s health, behaviour, needs or circumstances to the Supervisor immediately, either by telephone or in person.
Be a courteous representative of The SIL Company in all undertakings with service users, their carers and in the community during the hours of employment and ensure a positive public image is maintained throughout the community.
Competencies and experience required
Demonstrate past experience in working with people with disability and/or aged and their carers.
Demonstrate experience and performance in the key responsibility areas.
Must familiarise yourself with agency information management systems and policies and procedures which will change from time to time.
Demonstrate the ability to communicate sensitively and appropriately with and understand the needs of people with disability from a diverse range of socio-economic and ethnic backgrounds.
Demonstrate ability to work independently and report accurately to supervisor.
Excellent verbal and written communication skills including the demonstrated ability to communicate with service users and their carers, staff and range of service providers.
Must be capable in providing quality services that assists people to maintain their independence, dignity, cultural values and religious beliefs.
Ability and willingness to work in a diverse range of settings including but not limited to, a person’s home and other community settings.
Current driver licence
Reliable motor vehicle (including valid registration and full comprehensive third-party insurance).
Second language skills will be highly regarded.
An understanding of safe work practices, proven experience in the use of appropriate manual handling techniques.
