An exciting opportunity exists to lead and work as part of the Enhancing Supportive Community Care team (ESCC). This is a collaborative community-based multidisciplinary team which provides care to people with late-stage degenerative and chronic conditions, and disability. The service model is designed for people in the last 2 years of life with a focus on wellness and prevention of hospitalisation. Where you'll be working
This is a community-based position providing care to people in residing within NSLHD.
The office is based at Macquarie Hospital North Ryde.
What you'll be doing
The role will be accountable for the development and provision of the teams of high-quality clinical services in response to demand and client needs. The Department Head will provide leadership, management, and accountability for staff, and will ensure clinical practice is consistent and in accordance with evidenced based practice. The Department Head will carry a clinical case load.

Candidates will need to meet the following criteria:
Occupational Therapist or Social Worker eligible for membership of AASW. Social Workers and Occupational Therapists will need minimum experience as a Level 2.
What experience do you have working with people with late-stage degenerative, chronic conditions, and disability in the community.
What experience do you have in providing leadership and management of multidisciplinary teams
Describe your experience in the development and monitoring of quality improvement programs.
