JOB TITLE: Youth Worker EMPLOYMENT TYPE: Per-diem SCHEDULED HOURS: As Needed, Preferably Thursdays 1st shift and/or Saturdays 3rd shift PROGRAM/LOCATION: Grant House, Coventry, CT ABOUT THE PROGRAM: GRANT HOUSE A community-based group home which provides 24-hour staffing and a built in therapeutic component for 6 boys, aged 13 to 18, who have experienced trauma and loss and who have serious emotional and behavioral problems. Most of the boys referred to Grant House have been in hospitals or other residential treatment facilities before coming to CHR. The home allows the boys to live in a more normal, home-like setting within the community while providing the structure, services and treatment they need to be safe, to heal and to grow. DUTIES & RESPONSIBILITIES:
Maintains physical environment and program standards set by funders, accreditors, and agency policies.
Monitor self-administration of client medications in accordance with the agency medication policies and procedure as program may require.
Effectively utilizes appropriate communication methods/tools to effectively collaborate with coworkers and clients.
Teaches and assists all residents in areas of developmentally appropriate activities of daily living.
Assists/Manages administrative functions and tasks at the direction of the supervisor.
Prepares and facilitates educational and skill building groups and one to one intervention as
directed by the treatment plan.
Collaborates and coordinate care with providers and families as needed.
Assists residents in addressing medical and psychiatric needs.
Maintains documentation in compliance with agency and program standards.
Participates in staff meetings, supervision, trainings, and other meetings as assigned.
Additional duties as required.
QUALIFICATIONS: Education: High diploma or GED required. Experience: Two years’ experience working with a relevant population preferred. Licensure/Certification/Registration: Valid driver’s license required with good driving record. Why Join CHR? Award-Winning Workplace:
