Why does the job exist?
Work with CYFD JJS clients who are aged 18-21 to return to the community following a commitment to a secured JJS facility. Work is primarily done in the field; assisting clients with job search, locating housing, budgeting and life skills.
How does it get done?
Work with the Multidisciplinary Team while JJS clients are completing their commitment to create their transition plan to return to the community. Once clients are placed on supervised release Transition Coordinators work one on one with clients to return to the community.
Who are the customers?
Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care. Employment Requirements
Must possess a valid Driver's License. Pre-employment criminal background check is required and conditional pending results.
Working Conditions
Work is conducted in a office setting as well as the field working with clients in the community. Must have a valid drivers license and be able to drive state vehicles. Exposure to Visual/Video Display Terminal, personal computer and telephone usage. May have periods of sitting. Work will be with potentially hostile/aggressive clients
