Supervisor, Target Case Management
CCBHC Targeted Case Management Supervisor JOB TITLE: CCBHC Targeted Case Management Supervisor REPORTS TO: CCBHC Project Director DEPARTMENT: CCBHC We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance with a team at 1 Brooklyn clinic location with some travel in support of the CCBHC Project. SUMMARY: Experienced Case Management Supervisor needed for new CCBHC program dedicated to integrating mental health, substance use and medical care. The CCBHC program will provide care coordination, mobile crisis intervention and ongoing treatment in mental health, medical health and substance use in the Coney Island community. The Case Management Supervisor must oversee the quality-of-service coordination including, but not limited to: mental health, substance abuse and medical intakes and appointments; benefits; housing; community referrals; and social service needs. The person must be experienced with the intersection of mental health, substance abuse, and physical health needs. JOB DESCRIPTION: ● Works closely with the Executive Management Team to develop, launch and implement the CCBHC Targeted Case Management service. ● Provide strategic direction and leadership to the CCBHC staff, fostering a culture of excellence and collaboration with the Targeted Case Management team. ● Develop and implement policies, procedures, and protocols to ensure compliance with CCBHC program requirements and regulations for the Targeted Case Management team. ● Oversee the recruitment, training, and supervision of the Targeted Case Management team. ● Ensure the delivery of high-quality, evidence-based Targeted Case Management services. ● Collaborate with CCBHC staff to develop and implement treatment plans tailored to individual client needs and ensure a person-centered integrated approach. ● Monitor case management outcomes and program effectiveness, making adjustments as necessary to improve client outcomes. ● Develop partnerships with community organizations, healthcare providers, and other stakeholders to enhance service delivery and access. ● Monitor financial performance and identify opportunities for cost savings and revenue generation. ● Ensure compliance with billing and reimbursement requirements, maximizing revenue potential while maintaining program integrity. ● Establish and maintain quality assurance processes to monitor and evaluate the effectiveness of clinical services. ● Ensure compliance with all relevant regulations, accreditation standards, and reporting requirements. ● Participate in audits and reviews to ensure adherence to program requirements and standards.
● Implement agency training plan to improve cultural competency and trauma informed
care as they align to program requirements for CCBHC and Agency specific mission.
● Ensure CCBHC standards for services are being met and monitor progress with agency goals and objectives for performance improvement with quality indicators and fidelity to the model of core services. EXPERIENCE: ● 2 years in a supervisory case management role. ● 4 years in case management or care coordination preferred. ● Experience with adults with mental health and/or substance abuse needs. ● Knowledge of the intersection of mental health, substance misuse and chronic health conditions. ● Experience navigating behavioral health, substance treatment, justice impacted and social service system. ● Excellent knowledge of entitlements and other concrete benefits. ● Excellent written and verbal communication. ● Well organized with strong time management skills. ● Enthusiasm and self-motivation regarding identifying client needs and obtaining related resources. ● Able to work independently and as a member of a team. ● Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues. ● Proficient with word processing, electronic health records, and electronic communications. ● Able to meet program standards in documenting in client records ● Ability to work with challenging clients ● Demonstrated competency in written and verbal skills ● Able to meet program standards in documenting in client records ● Ability to work with challenging clients ● Comfortable with community outreach and home visits ● Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities. ● Bilingual skill in a language other than English is valued.
