Target Case Manager
CCBHC Targeted Case Manager JOB TITLE: CCBHC Targeted Case Manager- Benefits REPORTS TO: CCBHC Targeted Case Manager Supervisor DEPARTMENT: CCBHC We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance with a team at 1 Brooklyn clinic location with some travel in support of the CCBHC Project. SUMMARY: Experienced Benefits Case Manager needed for new CCBHC program dedicated to integrating mental health, substance use and medical care. Program will provide care coordination, mobile crisis intervention and ongoing treatment in mental health, medical health and substance abuse services. The benefits coordinator must conduct phone calls for client’s needs focused on screening for eligibility with public benefit programs, completing enrollment applications and tracking of application for approval and activation of benefits. The person must facilitate case management sessions, organize priorities of needs and engage the client with establishing goals. Requirements include attending supervision and training sessions and completing documentation of case management services and activities. JOB DESCRIPTION: · Enroll clients in Targeted Case Management service with a focus on benefit programs. · Assist clients with engaging in social service and benefit programs. · Help clients the use of the benefits with the various systems of care. · Assist clients with understanding and enrolling in health insurance and other eligible public benefits and entitlements. · Assist clients with understanding and appropriate use of approved benefits. · Help clients to identify, develop and pursue case management goals for a person-centered approach to case management. · Develop and prioritize steps in achieving case management goals with the clients. · Complete all case notes and assessments in a coherent and timely manner. · Ensure that all identified client needs, including but not limited to social determinants of health, are addressed. · Identify necessary social and community resources to address client needs. · Keep client insurance, demographic and contact information current in the client record · Develop, implement, track, and document discharge planning and termination when case management goals are achieved. · Identify other case management needs as appropriate and coordinate with other CCBHC providers, including any Health Home enrollment needs. · Work collaboratively with the CCBHC Interdisciplinary Team as well as with outside agencies. · Attend all staff meeting and complete all CCBHC and organizational trainings. EXPERIENCE: · At least 2 years in case management or care coordination preferred. · Experience with adults with mental health and/or substance abuse needs. · Knowledge of public benefit programs including, but not limited to, Medicaid, Medicare, Child Health Plus, Social Security, Food Stamps, CTA Bus Pass for Seniors, Housing, Unemployment, Emergency Funds, Rental Assistance, etc. · Excellent written and verbal communication. · Well organized with strong time management skills · Enthusiasm and self-motivation regarding identifying client needs and obtaining related resources. · Able to work independently and as a member of a team. · Excellent interpersonal skills, including being able to maintain a professional. temperament with both clients and colleagues. · Working knowledge of computer software and electronic health record documentation. · Demonstrated competency in written and verbal skills. · Able to meet program standards in documenting in client records. · Ability to work with challenging clients. · Comfortable with community outreach and home visits · Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities. · Bilingual skill in a language other than English is valued.
