The Advanced Care Center of Butler is a comfortable and home-like 29-bed skilled nursing center that provides high quality short-term rehabilitation services and comprehensive nursing care to help individuals successfully return to home.
Serves as an advocate to the Resident in a person center directed culture. The social worker is a part of the interdisciplinary team, responsible for identifying and developing care plans that address the psycho-social needs of the resident/patient. For assigned residents/patients, insure appropriate care plans, act as resident/patient advocate, staff education as appropriate. Develop goals for residents/patients and work with residents/patients to achieve these goals. Acts as liaison and provides educational support to residents/patients and their families.
- Obtain social history and social service assessment upon admission and initiates and develops a discharge plan within 7 days after admission.
- Accurately and timely complete designated sections on MDS forms and MDS quarterly assessments.
- Identify problems as related to residents/patients psychosocial well-being, and insures that those needs are being met.
- Updates discharge plans at least quarterly and as needed.
- Facilitates family meetings as needed, i.e. ethics, chaplaincy, etc.
- Assists residents/patients and families with executing powers of attorney financial concerns, referral and follow-ups for such areas as MA applications, billing issues, other insurances.
- Oversees accurate and timely completion of PASRR’s.
- Assists residents/patients with applications for various entitlement programs, such as PACE, energy assistance programs, etc.
- Establishes and maintains contact with residents/patients’ family (with residents/patient permission) to report on such things as, current goals, discharge planning and encourage participation in care planning.
- Provides counseling support as necessary, including strengthening communications between residents/patients, their families and significant others and staff.
- Processes transfers and discharges and documents on the medical record as appropriate.
- Attends and participates in resident/patient interdisciplinary care planning meetings.
- Performs other duties as assigned.
- Current License under the Social Workers, Marriage and Family therapists and Professional Counselors Act, or a Bachelors, masters or doctoral degree from an accredited school or program in social work or social welfare.
- 1 year experience in health or related field.
- Strong written and verbal communications skills.
- Working knowledge of computers, Microsoft Word and Excel required.
