Position Description:
GENERAL SUMMARY
Social Service Lead is responsible for ensuring the day-to day operations of both the Prescription Assistance Program (PAP) and the Social Work Department, working under the guidance and leadership of the Manager of Social Services. Social Service lead will ensure that ethical practices are adhered to while guiding both licensed social workers and medical case workers. The responsibilities include ensuring that case management services, clinical and non-clinical support are delivered effectively. Processes and protocols are effective, consistently applied, and aligned with the goal with meeting patient needs within an ambulatory care setting. Additionally, The lead will support policy writing, training, mentoring student apprentices and interns, providing clinical and non-clinical crisis support, assisting with program development, and performing other duties as assigned and outlined by Cooper Green Mercy Health Authority.
KEY RESPONSIBILITIES:
- Serve as a resource and mentor to licensed social workers and medical case workers, helping them navigate challenging cases while maintain high professional standards.
- Ensure social workers and medical case workers provide comprehensive case management services addressing social, emotional, and environment needs of patients.
- Monitor compliance across both departments, ensuring that all processes and services align with established standards and regulatory requirements.
- Foster collaboration between social workers, medical case workers, and PAP staff to ensure seamless, patient-centered care.
- Provide regular updates to the Manager of Social Services on operational performance, patient outcomes, and any ethical or compliance issues that arise.
- Assist in developing, updating, and implementing policies and procedures that govern the operations of the Social Work and Prescription Assistance departments, ensuring clarity and compliance with regulatory standards.
- Mentor and train student apprentices and interns, providing them with the necessary skills and knowledge to navigate both clinical and non-clinical aspects of social services. Guide their professional growth and ensure they adhere to curriculum in their program.
- Assist in developing and refining programs within the social work and prescription assistance departments, aimed at improving patient care, service delivery, and overall department efficiency.
- Act as a liaison between the social work team and other departments to ensure patients in crisis receive immediate and appropriate care.
- Monitor daily operations within the PAP and Social Work departments to ensure compliance with established protocols and regulatory requirements. Continuously assess and improve service quality.
- Collaborate with multidisciplinary team members, including medical providers, behavioral health, and administrative staff, to deliver patient care. Ensure that all services are coordinated effectively to meet the complex needs of patients.
