AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. The Resource Center Lead maintains oversight of day-to-day operation of the Resource Center team (Client Advocates). This position leads, supports, trains, and observes Client Advocates as they assist clients with navigation to resources both internally and externally (use of showers, use of phone/computers, obtaining ID’s, Birth Certificates, and enrollment into public benefits). This position also works closely with the Social Services Director as well as other department managers/directors/coordinators to ensure wrap-around care and efficiency of the Resource Center program. This position has direct client interaction.
Duties and Responsibilities:
Monitor day-to-day activities of Resource Center team and ensure the highest level of customer service to AHCH clients
Provide training, coaching, and support to a team of Client Advocates
Represent AHCH in meetings and trainings with community partners
Assist Social Services Director in tracking, and maintaining all performance goals of the Resource Center
Assist Social Services Director in collecting program data
Ensure quality of all Resource Center activities including client services (ID clinic, benefits enrollment, customer service, etc.) and documentation
Work in an integrated manner with all AHCH departments to ensure access to wrap-around services for AHCH clients
Work closely with Medical, Behavioral Health, and other Social Services teams to ensure Client Advocates understand pathways for clients to other AHCH programs
Oversight on adherence to HIPAA compliance in the Resource Center
Attend various meetings
Comply with all AHCH policies and procedures
Minimum Qualifications:
Previous experience working with those experiencing homelessness and/or those with SMI/CMI and/or substance abuse disorder
History of excellent customer service and ability to train that skill in staff
Willingness to be actively present during high-traffic Resource Center hours
Ability to work in a noisy environment with multiple distractions/disruptions
Ability to thrive in a complex nonprofit healthcare setting
High level of organization and adherence to deadlines
Ability to work in an integrated manner with all levels of organizational staff
High school diploma or equivalent
Preferred Qualifications:
1 year experience leading a team
1 year of leadership/supervisory experience in social services or behavioral health setting
Previous experience working in an FQHC or community healthcare setting
Associates degree in Human Services, Behavioral Health, or relevant field or commensurate relevant work experience
Bilingual Spanish/English
