Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. The candidate must be comfortable with traveling to clients in various parts of the city and transporting clients in our company vehicles.
Our organization practices the harm reduction approach and we need a candidate that understands and buys into this philosophy. Trauma informed care is another important philosophy of ours and we work with clients in this framework. We also believe in housing first and treating first, trying to have as many barriers to health care and housing removed as possible.
CSW caseloads can vary from 20-30 depending on client acuity and it is expected that the CSW will have a minimum of 60 encounters per month. Additionally, documentation of encounters must be completed within 72 hours of the visit date.
Duties and Responsibilities:
Assess individual needs of clients
Create individualized service plans with direct input from client in accordance with CCSS
requirements
Work in an integrated manner with other AHCH programs to optimize client care
Refer client to appropriate programs and agencies for coordination of wrap-around
services in the community
Participate in client chart audits for peer review
Provide direct service to clients with 60% of contacts conducted in home
visits/community visits
Perform duties in a trauma informed and client centered manner
Ensure accurate and appropriate documentation of all client contacts in Electronic Health
Record (EHR) within timeframe required by AHCH procedure and billing standards
Attend program-specific meetings and trainings including clinical supervision with
Behavioral Health Clinical Director
Attend organizational meetings and trainings
Must comply with all AHCH policies and procedures
Minimum Qualifications:
Ability to work in a client centered, trauma informed, and harm reduction manner to assist clients in meeting the goals that they develop
Strong understanding of HIPAA and how that plays into case work
BA in human services field or equivalent experience
Two years working with those with significant or chronic behavioral/mental health issues, those experiencing homelessness, and/or those with substance use disorder.
Valid NM Driver's license and clean driving record
Preferred Qualifications:
CCSS certification
Experience in trauma-informed care and harm reduction models
Bilingual or Spanish speaking
Past experience working with people with SMI/CMI
