Help Wanted: Overnight On-Call Licensed Practical Nurse (LPN) for Compassionate Hospice Care

Luna Del Valle Hospice is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our after-hours On-Call team. Position Overview:
As an Overnight On-Call LPN, you will play an integral role in supporting patients and families during after-hours. Working closely with our On-Call RN, you will assist with urgent patient visits, symptom management, medication administration, and other hospice-related needs. This role is crucial in ensuring continuity of care and support for patients and their loved ones during evening, night, and weekend hours.

Schedule:

• Monday through Thursday: 5:30 PM to 8:30 AM
• Friday through Monday: 5:30 PM to 8:30 AM (weekend coverage)
• Rotating schedule: One week on, one week off

Responsibilities:

• Provide after-hours patient care, including symptom management, wound care, and medication administration.
• Assist the On-Call RN with urgent visits and hospice-related needs.
• Communicate effectively with patients, families, and the interdisciplinary care team.
• Document care accurately and in a timely manner.
• Offer emotional support and ensure patient comfort during after-hours visits.

Qualifications:

• Valid LPN license in the state of New Mexico.
• Experience in hospice, home health, or palliative care preferred.
• Strong communication and interpersonal skills with a compassionate approach to care.
• Ability to work independently and handle urgent situations calmly and professionally.
• Willingness to work overnight hours and provide excellent care to patients and families.
