Responds to evening/nighttime incoming Agency calls from clients/families, referrals sources and other calls which cannot be held for office hours. Responds with telephone intervention and or makes home visits to assess and resolve problems being experienced by patients and families during evening/nighttime hours. Reports to Hospice Manager or Administrator On-call. Functions: 1. Is available as scheduled on call between the hours of 4 pm and 8 am weekdays, weekend, and holidays pertaining to client care and calls referred by the Agency's answering service. 2. Assesses client's needs and makes recommendations over the telephone as appropriate. 3. Is responsible for assuring that telephone interventions are safe, appropriate and effectively meets the client's needs. 4. Makes home visits as needed to provide nursing assessment and interventions to resolve the presenting problems. 5. Administers prescribed treatments, including IV services and teaches procedures to clients and/or families as ordered by the physician. 6. Completes required telephone and home visit documentation per Agency policy. 7. Documents in the clinical record in a clear, concise, manner in accordance with State and Federal Law and JCAHO Standards. 8. Receives report from staff or supervisors regarding real or potential client problems which may need evening/night intervention. 9. Reports evening/night client care interventions and outcomes to the supervisor, case manager or weekend nurse. 10. Contacts physicians for telephone orders. Completes verbal order form for confirmation of orders. 11. Obtains and transports laboratory specimens when indicated according to policy and procedure. 12. Consults with the clinical supervisors and/or Administrator On-Call, 13. Participates in mandatory in-service programs, including competency testing, online Learning for HCHB, and On-Call Team meetings as scheduled. Education and Experience: 1. Graduation of an NLN accredited school of nursing. 2. Currently licensed to practice nursing in Rhode Island and Massachusetts. If Massachusetts license is not held at time of hire, nurse will have until 1 year anniversary at VNHH to obtain Massachusetts licensing. 3. Two years of nursing experience. Qualifications: 1. Knowledge of and ability to apply nursing principles and practices. 2. Knowledge of IV therapy, Hospice, and pediatric care. 3. Knowledge of public health issues and the role of the community health nurse in the delivery of care. 4. Customer-focused ability to work with patients, families, physicians, and agency staff to maintains constructive working relationships. 5. Excellent problem-solving skills and the ability to exercise independent professional judgment. 6. Flexibility and ability to adjust schedule to meet needs of Agency and On-Call Team. 7. Computer Proficiency: including use of electronic medical record system (HCHB) and telephone system Job Specifications: On-Call coverage for Visiting Nurse Home & Hospice service areas.
