Position Summary The Mobile Crisis Intervention Case Manager is responsible to support the program clinicians (LMSW/LCSW). The primary goal of the Mobile Crisis Intervention team is to mitigate risk and avert the need for Emergency Department Evaluation and/or Inpatient Psychiatric Care for children and adolescents. Essential Duties & Responsibilities Direct Patient Care:
Supports clinical staff in completing clinical assessment and safety planning
Provides psychoeducation to parents and caregivers
Educates families and providers of additional community resources
Completes and follows up on referrals to community resources
Enters data in the Provider Information Exchange system (PIE)
Attends Weekly Rounds
Schedules and attends follow up visits with children and families
Receives and help triage calls from the 211 call center
Utilizes personal vehicle to travel to and from community visits
Documents timely in EHR
Work in collaboration with multidisciplinary team members including external partners
Participate in our high risk program
Demonstrates cultural competency and commitment to patients including LGBTQ+ community
Maintains excellent knowledge of community services and programs available to client population

Minimum Qualifications
BSW (Bachelors of Social Work) or comparable 4 year degree in Human Services or related field
Reliable transportation and valid driver’s license
Ability to have flexible schedule to include evening work hours
Comfort working with mental/behavioral health disorders
Must have excellent writing and communication skills, be highly organized
Must be able to work as a part of a team
Preferred Qualifications
Energetic, compassionate, quick thinking, creative, organized, team player,
Previous experience providing direct patient care is desirable
Knowledge of substance use disorders
Knowledge in working with LGBTQ+
