We are the best kept secret in Davidson, Davie, Rowan and Iredell which are the counties we serve. P.A.C.E. offers a wide variety of services to fulfill this mission such as medical services to medications, from nutritious meals to transportation – all personalized to meet the participant’s specific needs. Position: Registered Nurse (Home Care) ESSENTIAL DUTIES AND RESPONSIBILITIES: 10. Identifies health and safety hazards and initiates corrective action immediately including assuring the safety of participants under his/her supervision. 2. Receives and provides report from/to nurse he/she is relieving/coming on duty and records sufficient information to implement appropriate follow-up as necessary. 3. Visits participants in their homes to perform medical assessments, including a complete physical assessment and history of current and previous illnesses. Assesses participant needs and initiates development of individualized plans of participant care. 4. Develops an accurate comprehensive care plan based on each participant's assessment that establishes goals based on participant need and incorporates therapeutic, preventive, and rehabilitative actions. 5. Provides direct participant care. Provides health care instruction to participants as appropriate per assessment and plan. Gives instruction to Home Care Assistants. 6.Consistently works cooperatively with participants, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers. 7. Formulates and periodically updates care plan as necessary and as required by regulations. Monitors the effectiveness of the care plan and makes necessary revisions as participant status and needs change. 8. Listens to participant, family or physician concerns and initiates corrective action and/or reports concerns to RN Supervisor. Counsels the participant and family in meeting care needs. Provides clinical supervision and management of Nursing Assistants providing direct participant care. 9. Performs treatments in the home environment, including wound evaluation, dressing of wounds, etc. Dispenses medication at participant's home as ordered by physician. 10.Manages and monitors participant's medication and treatment adherence as prescribed by the physician. Coordinates appropriate clinical preventive and rehabilitative procedures. 11.Orders supplies and Durable Medical Equipment (DME). Educates participants on DME. 12.Educates participants and family members on disease process and preventive care. 13. In conjunction with Master Level Social Worker, responsible for discharge planning needs as part of care plan development. Implements prior to discharge of the participant. Serves as the community liaison for Carolina Senior Care and directs the contracts to meet the care needs of the participants. 14. Performs home visits after hours as necessary. Accepts after hours home care calls to resolve any home care related issues. 15.Accepts after hour’s on-call clinical rotation. Participates in orientation of new employees to facility policies and procedures and processes. 16.Records and reports changes in the participant’s condition to physician and RN Supervisor in a timely manner. 17. Orders, prepares and administers medications, enteral and parenteral nutrition and treatments as ordered. 18. Participates in orientation of new employees and evaluations of CNAs and LPNs. 19.Develops and coordinates home care service records to include times of service according to participant’s needs in the home. Coordinates assignments for home care assistants. Approves and monitors home care assistants’ schedules. 20. Performs other duties as assigned by supervisor. POSITION REQUIREMENTS: A. Minimal Education:
1. Graduate of accredited school of nursing.
2. Bachelor’s degree of Science in Nursing is preferred.
B. Minimal Experience:
1. Minimum of two years work experience as a registered nurse required.
2. One (1) year home care experience preferred.
C. Other Qualifications:
1. Current Nursing License.
2. CPR certified.
3. Ability to function with minimal supervision.
4. Excellent communication, organizational and human relation skills
5. Attention to detail, strong analytical and critical thinking skills are required for evaluation of medical conditions and coordination of care. Job Type: Full-time Benefits:
