Health Educator, Healthy Relationships JOB SUMMARY: The Health Educator, Healthy Relationships is responsible for providing comprehensive education and prevention services to individuals and groups, focusing on psychoeducation about Healthy relationships to the Peer Leadership Program. The Health Educator actively participates in outreach activities, such as tabling events, and is responsible for recruiting and retaining participants. Additionally, the role includes designing, facilitating, evaluating workshops, conducting intake/ behavioral assessment to examine risk factors, public relations and make appropriate referrals as needed. Collaboration is a key component of the position, as the Health Educator works with community organizations, healthcare providers, and internal teams to increase awareness, provide resources. Core responsibilities also include developing educational materials, conducting outreach, maintaining accurate documentation, and fostering an environment that encourages informed decision-making and well-being. EDUCATION AND QUALIFICATIONS:
MINIMUM: High School Diploma/ GED with at least three (3) years of professional experience in the field or
Bachelor’s Degree in health education or related field and one (1) year of relevant experience
MINIMUM: Two (2) years’ experience working with the population of focus.
Familiar with using EHR systems, plus
Up to 50% of traveling required.
Licensure: A NYS Driver’s License with at least a two (2) year clean driving record, preferred DUTIITES AND RESPONISIBLITES: Program Delivery & Facilitation
Deliver Healthy Relationship curriculum (20 HIV+ clients with 100 encounters).
Conduct all phases of the program curriculum as outlined.
Facilitate Peer Leadership Program (1 cycle of 10 weeks with 4 peers each, totaling 80 encounters).
Conduct Social Support Groups (8 HIV+ clients with 80 encounters).
Partnerships and Outreach
Create linkages with agencies that serve the HIV+ population, such as HASA, medical offices, SROs, and shelters.
Create and participate in 12 events at BAC, each with a minimum of 30 attendees.
Attend and participate in 9 external events (including some weekends), each with a minimum of 30 attendees.
Client Assessment & Outreach Services
Conduct Behavioral Assessments to evaluate clients' health risks and HIV/STD/HCV status.
Provide services and meet all program deliverables as stipulated in the program contract.
Conduct outreach via the mobile truck throughout Brooklyn, targeting individuals who utilize substances.
Coordinate services with community-based organizations across Brooklyn.
Develop and maintain relationships with alcohol and substance use detoxification, rehabilitation, and treatment centers.
Administrative & Reporting
Document all services within 24 hours in accordance with BAC recordkeeping policies.
Attend supervision, team meetings, and department meetings.
Maintain up-to-date, accurate individual paper and electronic case records.
Report serious incidents, allegations of abuse, and other sensitive situations, completing incident reports as per agency policy.
Comply with all applicable laws, regulations, and agency policies, fostering an ethical organizational culture.
CRISIS INTERVENTION & EMERGENCY RESPONSE
Execute emergency plans as outlined in the policy and procedure manual.
Administer CPR, first aid, or Naloxone as needed and provide crisis intervention.
OTHER DUTIES
Perform other duties as assigned based on program needs.
KNOWLEDGE, SKILLS, AND ABILITIES:
Minimum: Two (2) years in group facilitation, or providing educational curriculum
Exceptional written and verbal communication skills, including effective public speaking and interpersonal skills
Proficient in Microsoft Office (Word, Outlook, Excel, Power Point, etc.)
Previous experience in substance uses prevention Motivational Interviewing, Harm Reduction and Trauma-Informed Care approach, preferred
Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
Ability to learn computer databases e CR
Ability to meet deadlines, multi-task, prioritize, think logically, follow instructions and work in a fast-paced environment.
Must be willing to work weekends, nights, and long/flexible hours as required by the position.
