Compensation:
Approximately $20.87- $23.04 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
Overview of Role:
Enhance supports available to families with young children by improving patient engagement and building effective referral systems between Clinica Family Health and other social, behavioral, medical and/or community service agencies. The Case Manager-Pediatric Services will enhance systems to support young children and their families in accessing indicated services and supports. ESSENTIAL DUTIES AND RESPONSIBILITIES
Participate in a workgroup to develop standardized integrated care team workflows and trainings to identify and respond to positive developmental screens, including patient education, assessment of barriers to access referral resources, SDOH screening, placing referrals, and referral follow-up/navigation
Identify and maintain a solid understanding of local, state, and federal assistance and informational resources relevant to patient care to share with families and clinic staff
Conduct professional networking with community members and grant providers to enhance knowledge of resources
Support and maintain efficient screening and tracking systems to identify young children who may benefit from addition medical, behavioral, or social support resources.
Communicates solutions with organizational leadership and grant program staff to improve program and services
Participate in internal and external meetings to help problem solve barriers for families in accessing, utilizing and benefiting from services.
Monitor pediatric quality outcomes, conduct quality improvement and quality assurance efforts, support early childhood development screening workflows

POSITION QUALIFICATIONS: Education and Experience:
High school diploma or GED required.
1-2 years’ experience working in a community health setting or with underserved populations preferred.
BA/BS degree in Health/Human Services field in lieu of experience accepted.
Knowledge, Skills and Abilities:
Bilingual in Spanish and English required.
Ability to work in an integrated health care team.
Ability to work well under pressure with sensitivity to low socioeconomic status and historically marginalized communities.
