Healthy Living and Transitions Coordinator
The purpose of the role is to plan and coordinate the delivery of the National Child Measurement Programme (NCMP) in schools alongside key partners. It is working alongside the Change 4 Life programme to provide an appropriate intervention to those children, their schools, and a wider range of children and young people, in order to support behavioural change addressing issues associated with weight management. The role has the line management responsibility of a team of school health technicians who deliver the NCMP, hearing and vision screening, and the associated activities including health promotion in schools.
The role purposefully sits within the wider school nursing team where it is expected the role will form part of the MDT. The role will provide training, advice and support to school nurses around healthy lifestyles, working together in schools to improve early identification and support for pupils in need of healthy lifestyles intervention. The role will help embed a whole systems approach.
You will be passionate about promoting healthy lifestyles and delivering motivational change which is client centred. You will have a background in nutrition, exercise, public health and or weight management interventions, be an excellent team player with a willingness to learn and develop with us.
To work with the Clinical Service Lead, Business Support Officer and School Health Technicians to plan and deliver the National Childhood Measurement programme within Kensington and Chelsea and Westminster to ensure it is executed proficiently within both boroughs.
To line manage the team of school health technicians, including the delivery of supervision and annual appraisals.
To support and case manage children and young people in taking up and maintaining different health behaviour changes addressing weight management using evidence based education around nutrition, physical activity and behavioural change.

To deliver interventions using motivational interviewing, negotiating and empathic skills to work with families, children and young people to change behaviour and maintain the changes
To enter client data into the relevant computer-based data entry systems, to create reports from the database as necessary and to manage other information systems.
Build community links and partnership working to encourage new referrals into the service and maintain close working relationships with other community services.
To work within the wider school nursing service to be a resource, provide training, advice and support ensuring all staff are familiar with the key messages around healthy lifestyles, and to be a champion for this approach.
To work closely with the Change4Life Service to support engagement of children and families in the service.
To attend school events to help promote health lifestyles and provide health improvement messages e.g. coffee morning, schools assembly’s etc.
To actively plan and organise health promotion activities, such as health stalls, and input into community health events and campaigns.
To undertake any required background and preparatory work with schools during term times and continue to work with children and families outside of the school setting and in school holidays.
To participate within the multidisciplinary team to effectively support families who present with more complex concerns impacting on healthy lifestyles and in particular weight management.
Contribute to the contract performance report by submitting regular data and case studies to the Clinical Service Lead as part of the quarterly engagement meetings.
To support the wider workforce strategy which seeks to develop community champions, volunteers and local workforce engagement in the work delivered.
To support the wider coproduction strategy developed to support engagement of both families, children and partner agencies in shaping service development.
To audit work areas and participate in analysis and development activity as & when required.
