Ko wai koe | About You
This role provides Social Work Services to designated areas of work within Older Persons Health and Rehabilitation in-patient settings.
In this role, you will be responsible for:
  • Working in accordance with Social work protocols developed for the service in relation to inpatient assessment & rehabilitation, and associated community areas of work;
  • Social work bio/psych/social assessments, interventions, discharge and rehabilitation planning in accordance with professional social work standards and service requirements for cases referred;
  • Respond to health and disability needs that impact on the wellbeing of individuals and their family/whānau;
  • Undertake a range of statutory functions and responsibilities relevant to legislative requirements, this may include: child protection; mental health services and vulnerable adults;
  • Providing a consultation service to the service and community;
  • Action social work interventions, requests for information, and providing reports within adequate timeframes.

Your skills will include:
  • A flexible and adaptable approach to your work;
  • Experience with PPPR Applications
  • A robust understanding of rehabilitation
  • A robust understanding of hospital processes and procedures
  • Good understanding and experience with IPV/EAN
  • Good interpersonal skills, with ability to form relationships with clients and whānau under stress;
  • Registration under the Social Workers Registration Act 2003 with current APC;
  • 2+ years - experience within Physical and Mental Health as a practitioner.
