Social Worker (Kenepuru Community ORA)

Contract: Permanent, Part Time (0.95FTE / 38 hours per week)

Kōrero mō te tūranga - About the role
This opportunity is for a Social Worker to join the ORA Kenepuru Community Team. This position is part time (38 hours per week).
The Kenepuru Community ORA Team works with clients and their whanau who live in the Northern Wellington Suburbs and Porirua region (Ngaio to Pukerua Bay). This role is one of 2 social work roles working within our multidisciplinary team delivering Social Work assessments and interventions from a health perspective. Our main client groups are older adults and patients with a rehabilitation focus. The region we work in is multi-cultural and as a team we are striving to better service the needs of our Māori and Pacific Island communities.
Mō tō mātou rōpū - About our team
You will be part of the wider ORA Community Team of Allied health, Nursing and Medical colleagues. Collegial support with experienced clinicians is available and opportunities to collaborate is encouraged to improve patient outcomes. The team works with people 16 years and above with a range of physical disabilities, and age related or long term conditions to support their independence.
Moū - About you
This role would be the right fit for a Social Worker with at least 3 years post graduate experience in the general health setting. Experience of working alongside patients and whanau to understand their goals and needs, and an understanding of medico-legal complexity. Evidence based practice is at the heart of our service, so a commitment to keeping up-to-date on research and an interest in quality improvement activities would be an advantage. On the job training will be provided from our skilled and supportive team.
This role would suit a candidate with strong communication and organizational skills as well as an ability to build relationships and work well with clients, their whanau, colleagues in primary care, as well as other health service providers.

You will need:
  • New Zealand Social Work registration (meeting the requirements of the NZ Social Workers Registration Board) and a current APC * Current Social Work competence to practice certificate * A clear understanding of the health needs of our Māori and Pacific Island communities * Be able to undertake comprehensive social work assessments, and work collaboratively with others to provide integrated care. * Working visa/NZ residence (as appropriate) E pā ana ki a tatou - About us
