What you'll be doing
The Level 3 Social Worker in the Transitional Aged Care Program (TACP) delivers timely, effective services within the Geriatric Medicine Department. This role involves managing complex social work issues, providing supervision, and contributing to quality patient care.
- Manages the TACP social work team in Geriatric Medicine.
- Handles complex issues like elder abuse, child protection, and domestic violence.
- Provides supervision to social work staff and students.
- Conducts complex psychosocial assessments and interventions.
- Collaborates with multi-disciplinary teams for patient care.
- Engages in professional development and quality improvement activities.

Essential Requirements
Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
Selection Criteria
- Demonstrated experience in the delivery of social work services in Aged Care and or Rehabilitation, in a hospital or community setting, with demonstrated high level skills in psychosocial assessments, adjustment to illness / disability, new diagnosis, Elder abuse, Guardianship matters, NDIS and complex patients and familial presentations.
- Experience in leadership, including experience in staff and/or student supervision.
- Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills, and high-level computer literacy, and understanding of developing and implementing policies and procedures for service improvement and skills in providing training and education.
- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stake holders throughout all organisational levels.
- Ability to initiate, lead, complete and implement quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff.
- Current Driver’s licence valid in Australia and Participation in the After Hours Crisis on Call Social Work Service.
