Community Support Worker - In Home Care

Position Overview

Are you passionate about making a positive impact on the lives of individuals with disabilities? If so, we have an exciting opportunity for you. We are looking for dedicated In-Home Care Support Workers to provide high-quality, person-centered support to NDIS Participants. Your role will be crucial in helping them achieve their goals, enhance their quality of life, and improve their physical and mental health.

Key Responsibilities
As an In-Home Care Support Worker, your responsibilities will include:
  • Individualised Support: Providing a range of services tailored to meet the unique needs of NDIS Participants, such as housekeeping, grooming, toileting, meal preparation, medication management, and more.
  • Knowledge and Care: Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding restrictive practices and medication requirements.
  • Community Inclusion: Assisting Participants in participating in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.
  • Communication: Providing regular feedback to the Home and Community Support Manager regarding issues that require follow-up. This includes matters related to Participants, their families, external supports, fellow employees, and community access activities.
  • Compliance: Adhering to UFC's Code of Conduct, policies, and Quality Management System procedures to ensure the highest standards of care and support.
  • Safety and Wellbeing: Demonstrating a strong commitment to the safety and well-being of yourself, Participants, and others in the work environment.
About You
You will thrive and succeed in our team by bringing the following experience to this position:
  • Preferable 2 years experience supporting people in a related sector including with a disability, mental illness, community services, aged care, residential or rehabilitation environment
  • Drivers License and vehicle
  • Experience working within the In-Home Care environment
  • Ability to meet deadlines, work with minimal supervision and pressure.
  • Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
  • Understanding of WHS procedures
Desirable:
  • Qualification in community services, disability, mental health or related field
  • Qualifications in nursing, assistant nursing or registered nurse

  • NDIS Worker Screening Clearance (obtained from Service NSW)
  • Current First Aid Certificate
  • Unrestricted Australian Driver’s License and vehicle with Comprehensive Insurance
