The Care Coordinator will be responsible for:
Building strong connections with clients and their families
Assigning staff to clients based on staff qualifications and availability, as well as client preferences
Organise support for clients and monitoring and reporting on their progress
Referring clients to agencies that can provide additional help
Assessing client needs and resources for health, welfare and other facilities and services
Responding promptly to emails, enquiries and requests from clients, staff and management
Maintaining accurate records of incidents and feedback as required
Re-organising work priorities to meet the needs of a fast-paced environment
Supporting all clients to meet their goals
Conducting Client Care Assessments and Care Planning
Providing clinical guidance to other team members for client management
Discussing and promoting Right at Homes services with medical professions and referral sources
Ability to work in a fast-paced, dynamic environment
Qualifications
Qualified Nurse or Certificate IV
Commitment to a culture of teamwork, compassion, and service excellence
Strategic thinking abilities
Understanding of the dynamics of a commercial operation
Demonstrated leadership skills
Strong judgment, quick decision-making, and effective staff management
Excellent communication skills
Some understanding of education and accreditation standards, Continuous Improvement, Occupational Health and Safety (OHS), and risk management
Current National Police Clearance
First Aid and CPR certificates
AHPRA Registration (if applicable)
