About the role
We are seeking a values-driven and proactive AOD Case Manager to provide case management to young people aged between 15 to 24.
Reporting to the Regional Manager, this is a permanent, full-time position based in Chisholm, ACT.Salary and conditions are in accordance with the SCHADS Award Level 4.
You will successfully
  • Complete comprehensive intake/assessment, outcomes measures and risk and safety assessment/s for all young persons; including their history of AOD use, current and future risk.
  • Provide holistic, targeted evidence-based case management and care coordination to young people and their families accessing the service.
  • Utilise a range of therapeutic and evidence-based methods, including Trauma Informed Care.
  • Facilitate young people’s access to clinical alcohol and other drug treatment services and any other specialised services as required.
  • Develop and build a good working knowledge and positive relationships and partnerships with key stakeholders, ensuring to positively represent the service at all times.
  • Provide culturally inclusive and responsive support and intervention to all population groups accessing the service.
You will have
  • Minimum Tertiary Qualifications in Social Work or similar discipline, preferably Bachelor’s Degree or Higher (other relevant qualifications/experience within Community Services, Social Work or similar discipline will be considered).
  • Valid ACT Working with Vulnerable People Check.
  • Current ACT Driver Licence.
  • Eligibility for registration with peak body).
  • First Aid Certification,
  • Previous experience in case work or similar role working with young people is required.
  • Experience in providing case management and care coordination to young people impacted by their own, or other AOD use is desirable.
