About the Opportunity
The ACCP team is a small but mighty multidisciplinary team made up of Aboriginal Health Workers/Practioners, Care coordinators, Dietitians, Admin, CNC’s, Clinical Team Leader, Facility Manager and ACCP Program Manager.

The team covers the whole SWSLHD with 3 office/clinic sites across the district – Wellama (Bankstown), Budyari (Miller) and ACCP offices (Miller).
The team has a big focus on not only the wellbeing of our clients but also each other. The team work collaboratively in the provision of specialist services to Aboriginal patients and their families with chronic and/or complex needs in a culturally safe, patient centred way to facilitate optimal health outcomes.
As well as having access to our onsite clinics, we provide Integrated Team care, to assist Aboriginal and Torres Strait Islander people in South Western Sydney who are living with a chronic condition to navigate the Health system in partnership with their GP.
We do this through providing holistic, culturally appropriate health care to:
  • Improve health outcomes/lifestyles.
  • Increase early detection and intervention.
  • Increase knowledge and awareness through education of risk factors associated with chronic disease.
  • Work in partnership with Aboriginal Health Services, GPs and other healthcare providers.
  • To Close the Gap’ in life expectancy.
With the Ultimate Goal of Client Self-Management
We aim to 'Close the Gap' in health disparities for Aboriginal people by addressing chronic and complex health conditions in a culturally safe, patient-centered way.
Our service area spans the Dharawal, Gundungurra, and Darug Nations, covering Bankstown, Fairfield, Liverpool, Campbelltown, Camden, and surrounding Macarthur suburbs, as well as the Wollondilly and Wingecarribee areas.
