Location: Utica, NY
Blended Rate: $1600/wk
Shift: 5x5.7s - 40hrs - Days
Job Description:
We are actively searching for a dedicated and experienced Licensed Practical Nurse to join our team at a long-term care facility in Utica, NY. As an LPN in Long-Term Care, you will play a crucial role in providing comprehensive nursing care to patients with chronic illnesses, disabilities, or long-term medical conditions.
Responsibilities:

Provide direct and indirect patient care in accordance with the established nursing care plan and facility policies.
Administer medications, treatments, and therapies as prescribed by the attending physician.
Monitor patient vital signs, assess their physical and mental status, and document any changes or concerns.
Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans for each resident.
Assist in the coordination of resident admissions, transfers, and discharges.
Ensure nursing documentation is accurate, complete, and up-to-date.
Observe and report any changes in patient condition or behavior.
Promote and maintain a safe and clean environment for residents and staff.
Provide emotional support and education to residents and their families.
Adhere to infection control and safety protocols.

Requirements:

Valid and active Licensed Practical Nurse (LPN) license in the state of New York.
Minimum of 1-2 years of experience working as an LPN in a long-term care setting.
Knowledge of nursing principles, standards, and procedures.
Ability to assess, plan, implement, and evaluate nursing care.
Excellent communication and interpersonal skills.
Strong organizational and time-management abilities.
Ability to remain calm and focused under pressure.
Proficient in using electronic medical records (EMR) systems.
