Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in the comfort of their homes. Duties
Conduct thorough patient assessments to determine health status and care needs.
Develop, implement, and evaluate individualized care plans based on patient assessments.
Administer medications and treatments, including IV infusions, as prescribed by physicians.
Provide education and support to patients and their families regarding health management and home care practices.
Monitor vital signs and report any changes in patient conditions to the healthcare team.
Perform specialized procedures such as phlebotomy and Botulinum toxin treatment when necessary.
Collaborate with multidisciplinary teams to coordinate comprehensive patient care.
Maintain accurate documentation of patient progress and interventions in compliance with regulatory standards.
Ensure adherence to safety protocols and infection control measures during all interactions.
Qualifications
Current Registered Nurse (RN) license in the state of practice.
Previous experience in home health or similar settings preferred.
Strong knowledge of anatomy, patient assessment techniques, and critical care practices such as ICU experience is a plus.
Proficiency in administering IV infusions and performing phlebotomy procedures.
Experience with infant care or mammography is advantageous but not mandatory.
Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
Strong organizational skills with attention to detail in documentation and reporting.
Ability to work independently while managing multiple priorities in a home setting.
