Function: The licensed dietitian will provide nutritional services,dietary assessment and monitoring of in-center dialysis patients, home dialysispatients, and nursing home dialysis patients. Travel: Some travel is required between the islands of St. Croix and St. Thomas, U.S. Virgin Islands' facilities. Qualifications:
1 year experience in clinical dietetic nutrition
Has current state license
Maintains dietetic registration and continuing education hours as specified by the American Dietetic Association ADA
Demonstrate a working knowledge of the English language and ability to communicate verbally and in writing
Principal Responsibilities:
Nutritional care of the dialysis patient in accordance with physician diet prescriptions
Assist in the development of policies for nutritional input and care of dialysis patients
Be available for consultation with other team members regarding the nutritional status and care plans for the patient.
Provide nutritional material to staff
Conduct in-service classes for clinical staff as needed to educate them on the role of the dietitian, basic renal nutrition, psycho-social aspects of diet and pertinent nutrition referrals by staff to the dietitian
Maintain nutritional records on each patient instructed, and/or record follow-up progress notes in the dietary notes
Participate in team review of each patient interdisciplinary comprehensive assessment plan of care, to review patient progress, and to recommend changes in treatment based on the patient's current nutritional and dietetic needs
Assist other personnel through education and direction to support the dietetic services program by learning to deal effectively with nutritional and dietetic problems as they arise.
Assume responsibility for your own continuingeducation including participation in professional organizations related to ESRDpatient care management.
Participate in staff meetings or in-serviceeducation meetings when requested by the Executive Director.

A. Initiate nutritional care within one month ofadmission including: 1. Nutritional history 2. Working with physician for diet prescription 3. Nutritional Assessment 4. Nutritional care plan 5. Progress notes The Dietitian will attempt to conduct aninitial in-person evaluation, and at least one additional in-personevaluation each year. Additionalface-to-face consultation(s) will beprovided on an as needed basis. B. Assess the nutritional status and provideon-going follow-up care. C. Assess the patient’s nutritional and dieteticneeds (e.g.; review of monthly chemistries, participation in kinetic modelingstudies, evaluation of fluid status and shifts in body weight, evaluation ofdrug-nutrient interactions, etc. D. Recommend therapeutic diets. E. Counsel patients and their families on theprescribed diet, including nutritional principles, dietary plans, and foodselection and preparation, economics and adaptations required to best match theprescribed diet to the patient’s lifestyle. F. Document in the medical record an initialassessment of each patient and at least monthly progress notes which reflectall clinical nutritional and dietetic services provided. These shall include identification of currentproblems, current goals and the actions planned to achieve goals, and anevaluation of patient progress. G. Monitor patient response to nutritional regime,reviewing monthly lab work and counseling patient/family education. H. Assist in evaluating the nutritional componentsof home training education. I. Maintain confidentiality of patient’s medicalstatus and medical record. J. Coordinates patient care needs for patientsresiding in nursing facilities, with the nursing facility’s dietitian in aneffort to promote a continuity of care. K.
