Patient Service Navigator (Oncology): Saint Francis Cancer Center Physician Clinic

Job Summary: The Patient Service Navigator (Oncology) enhances the patient experience by initiating contact with patients after diagnosis and assists with the coordination of care and resources available throughout the treatment plan. This role collaborates with nurse navigators by supporting the initial screening process to determine barriers and facilitates timely access to care.
Minimum Education: Associate degree in Healthcare Administration or related field. 3 years of experience in lieu of education may be considered.
Licensure, Registration and/or Certification: None.
Work Experience: Minimum 2 years of experience in a healthcare setting using medical terminology. Experience in an oncology environment, preferred.
Knowledge, Skills, and Abilities: Working knowledge of medical terminology and understanding of processes within a healthcare setting. Knowledge and commitment to the health system’s mission, vision, and values. Effective verbal, written and interpersonal skills with the ability to obtain and convey information with a positive demeanor. Ability and desire to establish and maintain strong relationships with patients, families, and healthcare team members. Proven ability to make independent judgements and to evaluate complex issues. Ability to prioritize work assignments effectively with excellent organizational skills.
Essential Functions and Responsibilities: Serves as the initial contact to introduce services provided by the Saint Francis Health System Cancer Center’s Oncology Clinic. Acts as a patient advocate and functions as a point of contact for patients and families through the initial phase of care. Schedules patient's initial consultation appointment with the oncology physician. Collaborates with other healthcare team members to ensure necessary testing is ordered and completed prior to oncology consultation. Performs initial screening process with patients to determine barriers to care, such as transportation to appointment needs, financial and social support. Maintains accurate patient records by documenting all contacts, services provided, and outcomes; effectively communicating with patients and all healthcare team members. Assists patients with completion of necessary forms. Identifies any language barrier prior to appointment and plans for medical interpreter services, as needed. Develops and maintains strong working relationships with referral sources and community agencies and internal cancer care team.
Decision Making: Independent judgment in making decisions involving non-routine problems under general supervision.
Working Relationships: Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
