Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Responsibilities:
Conduct outreach to individuals (and families) who are experiencing homelessness, substance use, and/or co-occurring mental health disorders.
Provide harm reduction practices according to project objectives.
Screening individuals for recruitment to services.
Responsible for educating participants about their rights and responsibilities.
Referral to services and follow-up.
Assist in assessing the needs of program participants and creating service provision plans.
Directly engage with potential program participants and their networks.
Assist with coordination with internal and external programs.
Document outreach activities and contacts.
Responsible on retention of participants through various outreach and supportive activities.
Represent programs in different forums.
Coordinate with other team members for participant services or support.
Visit organizations and services providers to create collaboration relations for the benefit of the GBHI Project.
Driving the official or rented vehicle when necessary to complete project tasks.
Support satellite offices to ensure that they have everything necessary for the continuity of services to participants.
Supporting the work team in any task related to the project or the participants.
Prepare monthly reports.
Provide services for any activity that can promote access to homelessness, harm reduction (nocturnal activity, educational presentations/activity)
Job Requirements
GED, bachelor’s degree preferred (Work experience within the outreach field will be taken into consideration). Living/lived experience a plus.
Experience with conducting outreach with people that have experienced homelessness, substance use, and/or co-occurring disorders.
Basic knowledge about MAT/HIV/AIDS, Hepatitis-C and STDs in the Puerto Rican context
Willingness to learn and work with Evidence Based Practices / Interventions
Highly motivated to work with people who experience homelessness, substance use, and/or co-occurring disorders in an outreach setting.
Able to work closely with others in outreach team and office setting (team player)
Comfortable with using and learning to use technology as part of the outreach and office work.
Meticulous at maintaining records and confidentiality protocols.
