Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.


Benefits:

Compensation Range: $19/hr - $21/hr commensurate with experience

Overview:
Program Coordinator for SAMHSA Building Communities of Recovery Grant. Outreach based.

Responsibilities:
  • Coordinate all program services to individuals who are experiencing substance use, recovery and/or co-occurring mental health disorders.
  • Supervise personnel about task compliance, attendance, and performance.
  • Coordinates the monthly work plan.
  • Conduct weekly brief meetings to direct the work of the team.
  • Ensuring that evidence-based practices are properly addressed and documented.
  • Provide recommendations and follow-up if necessary.
  • Facilitate coordination and collaboration with other community-based organization and partners.
  • Ensure program integration with other agency services.
  • Assist in the coordination of all data gathering, analysis, and program reporting.
  • Prepare Progress Reports.
  • Collect monthly reports from the team, monitor service delivery trends, identify areas for improvement and make a corrective action plan.
  • Assist in completion of intake and follow up GPRA, enter data, and oversee data entry into the system and monitor project progress.
  • Represent organization in different forums.
  • Support the work team in everything necessary for the continuity of services.
  • Support the evaluator in the collection of information.
  • Report project progress or challenges to the project director

Job Requirements

  • Master’s in related field or related experience (life experience also taken into consideration)
  • Experience with direction or coordination of programs directed to people that have experienced substance use, recovery and/or co-occurring disorders in community setting.
  • Experience with supervision
  • Experience with Evidence Based Practices / Interventions
  • Knowledge about recovery, MAT/HIV/AIDS, Hepatitis-C, and STDs in the Puerto Rican context
  • Able to work closely with others in outreach team and office setting (team player)
  • Proficient and efficient with multiple computer software programs
  • Excellent written and verbal communications skills (in both Spanish and English)
  • Meticulous at maintaining records and confidentiality protocols
  • Outreach experience
  • Up to date driver’s license
