MDS Coordinator Job Description Job Summary
The MDS Coordinator plays a crucial role in the healthcare team, responsible for overseeing the assessment and care planning process for residents in a nursing home setting. This position requires a thorough understanding of the Minimum Data Set (MDS) process and compliance with federal and state regulations. The ideal candidate will possess strong organizational skills, attention to detail, and the ability to work collaboratively with interdisciplinary teams to ensure optimal patient care.
Responsibilities
Conduct comprehensive assessments of residents using the MDS tool to gather necessary clinical information.
Collaborate with nursing staff and other healthcare professionals to develop individualized care plans based on assessment findings.
Ensure timely completion and submission of MDS documentation in compliance with regulatory requirements.
Maintain accurate records and documentation related to resident assessments and care plans.
Monitor resident progress and outcomes, making adjustments to care plans as needed.
Provide training and support to staff regarding MDS processes and documentation standards.
Stay updated on changes in regulations, coding practices (including ICD-10), and best practices related to managed care, critical care, and acute care settings.
Utilize electronic health record systems such as Athenahealth, Cerner, or e Clinical Works for documentation and reporting.
Qualifications
Proven experience in a healthcare setting, preferably within a nursing home or long-term care environment.
Strong knowledge of physiology, managed care principles, and critical/acute care practices.
Familiarity with MDS processes, including assessment tools and regulatory requirements.
Proficiency in electronic health record systems; experience with Athenahealth, Cerner, or e Clinical Works is highly desirable.
Excellent communication skills with the ability to work effectively within an interdisciplinary team.
Strong organizational skills with attention to detail for accurate documentation and compliance.
Ability to analyze data and make informed decisions based on resident needs and regulatory standards.
Job Type: Full-time Pay: $70,000.00 - $90,000.00 per year Benefits:
