Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including home community care and hospice care. The LPN will work closely with registered nurses and other healthcare professionals to ensure the well-being of patients through effective assessment, treatment, and education.
Responsibilities
Conduct patient assessments to determine health status and needs.
Administer medications and treatments as prescribed by physicians.
Provide family planning education and support to patients.
Assist in infection control measures to maintain a safe environment.
Collaborate with healthcare team members to develop and implement patient care plans.
Maintain accurate patient records using e Clinical Works or similar electronic health record systems.
Support patients with internal medicine needs, including monitoring vital signs and managing chronic conditions.
Assist in procedures such as dermal fillers and dialysis when required.
Provide compassionate end-of-life care in hospice settings.
Skills
Proficient in patient assessment techniques and documentation practices.
Knowledge of family planning methods and community health resources.
Strong understanding of infection control protocols.
Experience with internal medicine practices is preferred.
Ability to lift patients safely and assist with mobility as needed.
Familiarity with e Clinical Works or similar electronic health record systems is advantageous.
Experience in providing hospice care is a plus.
Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare teams.
