Full Job Description: CDDC is seeking a compassionate and experienced HCBS- Day Service Coordinator to oversee and manage services within our day service center. The Day Service Coordinator will focus on managing service plans and liaising between families, staff, and OPWDD to ensure individuals receive high-quality care that meets regulatory standards. Position Overview:The Day Service Coordinator is responsible for managing and overseeing services provided to individuals at the day service center, ensuring compliance with OPWDD regulations. The role focuses on coordinating care plans, monitoring staff documentation, and maintaining communication with families to ensure service satisfaction. Key Responsibilities:
Develop, implement, and monitor individualized care plans for individuals attending the day service center.
Ensure that services provided align with OPWDD regulations and meet the individuals' needs and goals.
Review and monitor staff documentation, including progress notes, to ensure accuracy and compliance with OPWDD and agency standards.
Collaborate with families, interdisciplinary teams, and external providers to coordinate services and ensure seamless communication.
Attend Life Plan meetings and ensure care plans are updated in accordance with meeting outcomes.
Ensure all documentation, including incident reports and billing, is completed and submitted timely and accurately.
Conduct monthly check-ins with families to ensure satisfaction with services.
Report and document all incidents according to OPWDD and agency guidelines.
Maintain regular communication with day service staff to ensure that services are delivered according to the care plans and that any issues are promptly addressed.
Qualifications:
Minimum of 3 years of experience working within the OPWDD field, with a focus on service coordination.
QIDP (Preferred)
Strong communication and collaboration skills.
Ability to work independently while managing multiple responsibilities.
Experience using data collection software.
Strong organizational skills and attention to detail.
