HCBS Care Coordinator
Share Care USA is looking for a mature and experienced Care Coordinators to run and manage a case load of clients.
Valid driver’s license, valid registration, valid car insurance, valid inspection sticker, working seatbelts, air and heat in car.
HCBS Care Coordinators:
1. Staff all shifts within the case load. The Care Coordinators will be on an an After Hours On Call rotation. Will fill in as DSW if needed.
2. The Care Coordinators will need to live in region to accommodate emergency staffing requirement and rotating On Call.
3. Reviewing all time sheets and progress notes for errors or corrections.
4. Assist with client financials as required such as but not limited to (collecting receipts timely, dropping off rent checks, explaining finances to clients, and teaching client about spending patterns).
5. Team meetings weekly and/or as scheduled.
6. Complete all individualized service plans for clients.
7. Participate in the planning of services for all clients.
8. Provide weekly / quarterly face-to-face visits with all clients as evidenced by signed face-to- face visit forms.
9. Provide bi weekly / monthly phone contacts with clients as evidenced by phone record logs.
10. Communicate with support coordinators when necessary in order to maintain current data on all clients.
11. All other assigned tasks.

Pay: $15.00 - $17.00 per hour
