- Ability to work well under pressure meeting deadlines with efficiency and consistency while maintaining appropriate self-care with a balance between good physical and mental health and the job requirements.
- Such specialized training and/or certification in mental health services or related field as may be required in order to provide adjunctive services, i.e. Children Case Management Certification; BHRS, S.O.C. training and updates, state mandated trainings etc.

- Current Oklahoma Operator's Driver's License and a good driving record acceptable to the insurance company insuring the agency vehicles. Must provide own transportation.

ESSENTIAL DUTIES AND RESPONSIBILITES:
- The Family Support Provider (FSP) is to provide targeted support, advocacy, education, behavioral support and mediation/collaboration type services within a system of care that is child-centered and trauma-sensitive with the needs of the child and family dictating the types and mix of services provided, to assist in preventing an out-of-home placement. Domain A, B, C and D.
- The FSP II function will be initiated through a non-diagnostic, interview with the client and his or her family to assess service needs. Domain A, C and D.
- Responsible for providing direct/indirect advocacy, behavioral support, education, case management and/or rehab services to maintain agency productivity requirement of 25 hours billable service per week. Domain D and E.
- To be effective, the FSP II will work as a team with the Care Coordinator, clients in their homes, school/work and communities while incorporating the entire family and support team in the treatment program, with a focus on client/family strengths as well as the child's age, developmental level, and cultural heritage. Domain A, B and C.
- Will schedule, coordinate and facilitate wraparound / family teams. Domain D and E.
- The FSP II will maintain close communication with the Project Director, Care Coordinator, Therapists, Physicians, etc.. and other clinical and administrative staffs in order to assure the highest quality of services are delivered to the client and his or her family. Domain B and C.
- Complete initial care plans, strengths and culture discoveries and intakes as needed, update and modify care plans as needed. Create a family plan and course of action based on individual needs of a family. Domain C, D and E.
- To provide targeted support and advocacy services (behavior management, daily living skills, social skills); linkage with community supports; referral to local needed services; advocacy; service coordination; and follow up on behalf of client and their family who are experiencing emotional and/or behavioral challenges that are significantly interfering with their success at home, school, work or in the community. Domain A, B, C and D.
- To provide services consistent with the Sanctuary Model assuring that care is child-centered, trauma-informed with the needs of the child and the family directing the types of services provided and referred to. Domain A, B and C.
- To assist in preventing an out-of-home (or school) placement and/or in facilitating discharge from an inpatient or residential treatment facility. Domain A, B, C and D.
- Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs; Complete flex-fund requests when necessary. Provide transportation for clients and families to appointments, trainings, and meetings when a bus or cab is not appropriate. Domain D and E.
- Facilitate or co-facilitate referral Teams meetings and may type the agenda. Domain D.
- Contact referred families and the referral source for screening and notifying providers of care team meeting dates within appropriate time frames. Domain D and E.
- Provide the data entry clerk with the new family teams lists and provide the clerk with the approved referrals. Domain D and E.
- Responsible for teaching about and implementing regular community meetings and safety plans with client(s) as part of commitment to the Sanctuary Model of client care. Domain A and C.
- Decrease of inpatient stay through alternative service planning and coordination of follow-up referrals. Domain A, B and D.
- Provide targeted and continuous follow-up to appropriate, least restrictive and least intrusive services. Domain A, C and D.
- Provide basic social skills training in such areas as: anger management, feelings identification, positive interpersonal communication, impulse/self-control, healthy relationships, and other related coping skills, using a best-practice curriculum. Domain A, C and D.
- Be a member of the family team to assist in the strengths assessment and care plan development. Recognize the individual child client and his/her family has different barriers to social, economic, and employment opportunities with varied levels of need to overcome these barriers. (The goals for each juvenile client as identified in the family team will be tailored to reflect a juvenile client's particular needs in order to realize her/her full potential). - Coordinating services between agencies and developing cooperative working relationships with agencies serving children and families. Domain A, B, C and D.
- Provide immediate crisis intervention on a 24-hour basis. Domain A, B, C and D.
- Maintain close communication with immediate supervisor, team members, and other clinical or administrative staff in order to assure the highest quality services are delivered to the person served and his or her family. Domain A, B, C and D.
- Be responsible for seeing that all services provided to any client to whom he or she is assigned will be properly charted and reported within agency standard timelines (48 hours after providing services). Domain A, C and D.
- Responsible for submitting accurate and timely monthly and quarterly service numbers as required or requested. Domain E.
- Support and promote a service environment for individuals in need of behavioral health and/or co-occurring services that is trauma-informed, recovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives. Domain A, C and D.
- Work with the families and Care Coordinator through a non-diagnostic interview to assess service needs; Perform a basic screening to determine whether COD might exist and be able to refer the consumer for a formal diagnostic assessment for co-occurring disorders. Domain A, C and D.
- Be able to engage the client in an emotionally intelligent manner to enhance positive rapport and facilitate future positive interactions. Domain A and C.
- Have the skills to de-escalate clients who are agitated, anxious, angry, or in other vulnerable emotional states. Domain A, C and D.
- Mange a crisis involving a client, including a threat of suicide or harm to others. Domain A, C and D.
- Refer a client to the appropriate mental health or substance abuse treatment facility when needed and follow-up to ensure the client received needed care. Domain A, C and D.
- Have knowledge of psychotropic medications and their actions. Domain A, C and D.
- Attend weekly staff meetings consistently as well as other departmental, clinical, or agency meetings when scheduled. Domain E.
- Attending or speak at conferences as needed and assigned by the Project Director. Domain B, C and E.
- Participate in the children's department on-call crisis rotation as needed or requested. Domain B, D and E.
- Other duties as assigned by the Wraparound Project Director.

Physical Requirements: Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures/programs, enter data to various programs. Must be able to climb stairs and access non-handicapped equipped residences/facilities.
HIPAA: This position is permitted to locate information necessary to follow through with client care or provide authorization for release of information outside the agency. Information should be requested from Medical Records staff initially or WITH authorization from administration, retrieve necessary information without supervision. Level 2 access.

HOURS: 40 hours. Full-time variable hours. Some evenings and/or weekend work will be required.
This position has a weekly productivity requirement of 25 hours. All documentation must be entered within 2 business days of service being provided.
