Job Summary: To work as a proactive member of the Access Point RI team, providing meaningful personalized supports that encourage participants and their families to experience personal growth, achieve their personal goals, and reach their fullest potential. Teamwork and Participation:

Work collaboratively with agency Directors and Coordinators to coordinate supporting DSP efforts.
Essential Duties and Responsibilities:
Ensure that all individuals and families supported are treated with dignity and respect.
Provide supports and services consistent with and as outlined in the individuals personal support plan.
Support individuals and their families in developing a positive self image.
Treat each individual and their family as persons who have unique needs and wants.
Support individuals and their families to function to their fullest potential.
Support individuals and families in exercising control around selecting services and supports they receive.
Provide daily and ongoing support as needed in areas of personal needs such as lifting, personal hygiene, and nutrition.
Support individuals and families in participating in their community in ways that are meaningful to them.
Encourage individuals and their families to maintain positive relationships.
Work in conjunction with the individual and their family in developing an environment conducive to learning.
Complete preparation materials and actively participate in the semiannual planning meetings. For example, status updates, plan renewal, and team meetings. Gather and submit documentation and data related to achieving individual goals outlined in the personal support plan.
Attend in service training as determined by the Program Director or Coordinator.
Support and carry out all team decisions consistently and appropriately.
Ensure the optimal health and safety of individuals receiving support.
Administer medication as appropriate.
Maintain open and ongoing written and verbal communication with the family supervisor on a biweekly basis.
Adhere to all safety policies, including safe driving and seatbelt policies.
All other duties as assigned or as identified in the personal support plan and individual profile.
Qualifications:
High School Diploma or GED
Minimum 1 year of experience working with individuals with developmental disabilities.
A valid drivers license and acceptable driving record are required.
Willingness and ability to complete CPR, Non Violet Crisis Intervention, HIPPA, and Human Rights training required.
Ongoing access and provision of an insured automobile are required.
An Associate Degree in Human Services or a related field and one year of supervisory experience are preferred.
Necessary Knowledge, Skills, and Abilities:
Ability to communicate effectively both in written and oral formats.
Ability to use varying degrees of independent judgment and discretion.
Ability to read and comprehend policies and instructions.
Essential Physical Demands:
Must be able to exert up to 100 lbs. with two people and lift up to 50 lbs. independently. Physical demands are more than sedentary work as the position frequently requires walking, standing, reaching, fingering, stooping, bending, pushing, and pulling.
Work Environment:
Participation indoors as well as outdoors during all seasons.
