Overall job purpose
The post holder will be based across Specialist Children Services and the Adult Learning Disability Service. These are multi-disciplinary teams where the ability to work with other professionals is paramount. They will manage an allocated caseload of children of transitional age which can include Assessment and Social Care interventions as required.
They will plan and co-ordinate activities to support the effective transition of disabled young people to adult services to maximise the independence, wellbeing and safety of the young people of Anglesey going through transition to adult services.
The post holder will do this by undertaking assessments of need with young people and their families / carers using the Department’s procedures, and where required will undertake separate assessments of the needs of the person’s carer(s). They will develop and design transitional plan and where appropriate care and support plans.
They will plan creatively with individuals, maximising assets, reducing risks, and reducing dependency on long term service provision. This will include the provision of information and advice, supporting people to access universal community resources, and accessing statutory care and support services as required as the transition to adulthood.
