New Directions is a social service agency offering a wide range of resources and services that provide supports in a variety of settings and foster people’s hopes and dreams in their communities.
The Coordinator for Community Supports and Outreach/Shared Living is responsible for overseeing staffing at supports at Shared Living locations, as well as assisting with expansion of the service area as needed. Serving as a primary point of contact for Supervisors, and responsible for providing supervision and evaluation, the Coordinator oversees the development of staff teams, and engages with the Community Support and Outreach team in the planning, consultation, communication and implementation of various projects and initiatives within the service area.
Overview of the work you will do:
Person Led Support Coordination
- In collaboration with service area leadership, assists the Shared Living Supervisor to plan and develop individualized and group supports that focus on personal interests and increase experience, exposure and education.
- Use Personal Outcome Measures as a tool to determine what quality of life looks like for each person and uses this information to inform support plans.
- Builds positive, respectful and non-judgemental relationships with people supported and their families, and models same for Supervisors and CRWs.
- Using a ‘Community First’ mindset, helps find resources that can be used by people supported to increase connections with others and find meaningful ways to contribute and solve problems.
Leadership
- Provides direct supervision to Shared Living Supervisors, including managing performance and areas of development.
- Mentors Shared Living Supervisors in direct supervision of staff, including managing performance and areas of development
Facility Management
- Oversees building maintenance for all Shared Living sites, including working with building management and coordinating repairs with New Directions Facilities and Maintenance as needed.
- Work collaboratively with Community Support and Outreach leadership to identify facility needs, including acquiring new apartments for expansion.
- Build positive, cooperative relationships with management companies, and seek new opportunities to partner with management companies to expand Shared Living Services.
- Ensures people supported in Shared Living understand their tenant responsibilities
Financial Management
- Operates Shared Living location within the budget allocated.
- In absence of Manager, prepares and signs off on financial forms (funding sheets, cheque recs etc).
- Assists Manager with budgeting process and on-going financial review of Shared Living
Administration
- Maintains files for people supported in Shared Living.
- Maintains staff supervision files, and mentors same for direct reports.
- Completes all forms and plans as needed for the intake and referral process.
- Assists Shared Living Supervisors to complete all forms specific to people supported as needed
- Scheduled leadership team rotation for service area emergencies/crisis response.
The successful candidate will bring the following:
- Minimum of five (5) years of experience working in the adults with disabilities service sector.
- Post-Secondary education (diploma) in Disability and Community Support from a recognized post-secondary institution. A combination of education and experience may be considered.
- Previous supervisory experience, while maintaining the ability to work independently on projects and independent tasks.
- Clear and concise verbal and written communication skills.
- Proficiency in Microsoft Office suite, including Outlook, Excel, Work, Power Point, and Share Point.
- Fluency in American Sign Language is considered an asset.
The salary for this full-time, 70-hour bi-weekly position is $33.90 to $42.71 per hour, dependent upon education and experience. A full benefits package (which includes Health and Dental Benefits, Long Term Disability Benefits, and a Pension Plan) is provided to regular full-time staff.
The successful candidate will be subject to Criminal Record and Adult/Child Abuse Registry Checks, and Prior Contact Check. A satisfactory Driver’ Safety Rating is also required.
