The Community Services team in North Metro Perth supports people living with disability through community engagement. The difference you can make includes supporting our customers who are all very individual and have varying needs. You will need to be prepared to support customers within an aged care facility supporting with personal care and medical appointments. Please review the rosters to conform that you can commit to the hours included before applying. Proposed Roster - Community - Female - 17 hrs.pdf Key duties include:
Delivering a wide range of domestic supports within the home and community;
Supporting customers to engage in community-based activities;
Supporting person-centred plans that identify a customer’s life goals;
Ensuring positive behaviour support plans are followed; and
Administration duties, expense and mandatory reports.
What you’ll need?
Well-developed interpersonal skills with the ability to build effective working relationships;
Ability to follow Person Centred plans and assist people to achieve their goals and outcomes;
Previous experience with manual handling and medication administration;
Willingness to assist with personal care and domestic duties experience;
Ability to manage situations as they arise with patience and maturity;
A Valid drivers licence and a willingness to use your private vehicle (aligned with Activ’s motor vehicle allowance rebate).
