About your role:
- Ensure patients and other staff are treated fairly and with mutual respect and understanding, regardless of gender, cultural, religious, ethnic and linguistic backgrounds.
- Provide advocacy to address client need and ensure equity and access to services, including, and not limited to, the utilisation of existing links with the Public Guardian, QCAT, Residential Aged Care Facilities (RACF), NDIS and other agencies to benefit patients and families.
- Participate in the clinical assessment and intervention planning for service recipients from a Social Work and multidisciplinary team perspective, ensuring timely identification of client's needs, service provision, and discharge planning within the work unit or professional team.
- Lead staff by fostering and committing to patient safety and quality in the delivery of health care by maintaining and evaluating safety and quality practices and initiatives.
