Job Description: We are seeking a highly skilled and compassionate Support Coordinator/Case Manager to join our team. The ideal candidate will be responsible for helping participants navigate their NDIS plans, coordinating supports, and ensuring that they receive high-quality services. You will work closely with clients to assess their needs, develop support plans, and connect them with the necessary resources and services. Key Responsibilities:
Assist clients in understanding and implementing their NDIS plans.
Coordinate a range of support services, including personal care, therapeutic supports, and community participation activities.
Develop, review, and update individualized support plans based on clients' needs and goals.
Collaborate with other healthcare and social service professionals to ensure comprehensive care.
Advocate for clients to ensure they receive appropriate and timely support.
Provide ongoing support and guidance to clients, helping them overcome barriers and achieve their goals.
Maintain accurate and up-to-date client records and documentation.
Monitor and report on the progress and outcomes of support plans.
Essential Requirements:
Diploma level qualification in a relevant field (e.g., Social Work, Disability Services, Community Services).
Minimum of 2-3 years' experience in the disability sector or a related field.
Strong understanding of the NDIS and its processes.
Excellent verbal and written communication skills.
Demonstrated ability to work independently and as part of a team.
Compassionate, empathetic, and dedicated to making a positive difference in the lives of individuals with disabilities.
Clear police check.
Fully vaccinated against COVID-19.
Preferred Qualifications:
Experience in case management or support coordination within the NDIS framework.
Proficiency in a second language.
