Key Responsibilities:
Deliver case management services aligned with the Uniting SA framework for homelessness.
Offer goal-oriented services to meet client and customer needs.
Manage referrals, assess needs, and provide appropriate services.
Respond to client enquiries and address complaints.
Facilitate linkages and referrals to other agencies when needed.
Identify priorities and manage high workloads to meet deadlines.
Execute engaging programs for relevant service outcomes.
Provide information and referral pathways for self-management support.
Skills & Experience:
Essential: Hold a relevant Human Services qualification or equivalent degree.
Desirable: Training related to complex needs in the Homelessness sector.
Proven experience in diverse intervention models within human services.
Skilled in crafting detailed reports.
Expertise in case planning and complex case management for marginalised individuals.
Hands-on Case Management experience, covering assessment, planning, referral, and advocacy.
Provide diverse services, recognising the impact of complex needs in homelessness.
Collaborate with a strengths-based approach for those at risk or experiencing homelessness.
Efficiently coordinate with government and non-government agencies.
Manage databases, and analyse data for reporting and service improvement.
Report on trends and issues for those at risk or experiencing homelessness.
